Any new television documentary needs to explore this and ask did & searching questions. Those guilty of such behaviour need to be publically named & shamed so that the public know who they are, and in order that any potential future employers know of their history of wrongdoing (particular when that wrongdoing included breaking the law). All we want is for HMRC to be open and act with integrity.
